Important Notes


• Wellness Exams - A preventative benefit that most insurance providers will cover once a year if that is all that the patient has done. Wellness exams generally do not cover anything else that is needed to be addressed (ex: refills/labs etc.).
• Mammogram - For breast cancer screening, we offer mammograms. This is a preventative benefit and insurance will pay if that is all that the patient has done. It will not pay if anything else is addressed (ex: refills/labs etc.).
• Texas Health Step - Medicaid requires that the patients have labs run at different ages and therefore the patient is required to show up 30 minutes early to their appointment so that they can have lab work done prior to their visit, shot cards are required.
• Vaccine-only Visits - Patient shot cards are required so that the nursing staff can verify whether vaccines are needed. We keep a copy of the card on file for us to document and enter into IMMTRAC (state vaccine agency). We are not able to pull them out to rewrite for patients on the same day. There is a $10 fee to purchase a new shot card. An appointment is not necessary as they are done as walk-ins on Tuesdays and Fridays from 2-4 p.m., year-round.
• Sport's Physicals ($49.70) & Daycare Physicals ($68.40) - Insurance does not cover these services and payment is the responsibility of the patient/parent/guardian.
• Head start physicals ($68.40) - Commercial insurance does not cover these and are the responsibility of the patient/parent/guardian. Medicaid, however, does cover.
• New patients - Anyone wishing to establish care at our clinic will require a 40-minute appointment as more information is necessary for the provider.
• Minors - Anyone other than a biological parent bringing a minor to an appointment requires written consent from a biological parent. Legal guardians/adoptive parents/foster parents must present legal documents at the time of the visit.
• Insurance will not cover same-day visits to the ER and clinic. Patients can pay out of pocket. Any tests/labs ordered at the time of the visit will also be an out-of-pocket expense to the patient.
• HMO insurances require that patients be locked in with a specific provider. Visits with any other provider are not covered. Patients may make changes to the primary care provider (PCP) through their insurance and must be done prior to the visit.
• Medicare Part A insurance - only covers inpatient services. It will not cover clinic visits and lab work.
